Adult Day Services
The Town of Brookhaven provides a safe, secure and supervised environment for its frail elderly residents who need assistance with activities of daily living and/or cannot safely be left home alone. We serve a diverse population of senior citizens from all over the Township, at 2 convenient locations in Mt. Sinai and Yaphank.
Our Social Model program aims to delay or prevent institutionalization by providing alternative care that includes social activities, meals and recreation designed to improve the physical, mental and social well being of participants. An individualized plan of care is designed for each participant. We offer support services for both clients and their family caregivers.

Eligibility:
Applicants are assessed on an individual basis after meeting the following criteria:
- At least 60 years of age
- Unable to use existing senior citizen programs
- Under the care of a physician
- Able to handle his/her personal hygiene needs
Cost:
Fees include all activities plus a continental breakfast, hot lunch and afternoon snack
$42 per day with transportation
$33 per day without transportation
$50 per day for non-Brookhaven residents (Transportation unavailable for non-residents)
Brief RESPITE
Our Brief Respite Program is available to residents 60 years or older, caring for a relative in their home, who is also age 60 or older and cannot be left alone due to extreme frailty or dementia. Limited to caregivers who have no other means of providing supervision for the client. Provides temporary custodial care for up to 4 hours, one to two days per week. No personal or medical care is provided. Aide cannot transport client.
Program Hours: Monday – Friday 9:00 am – 4:30 pm
Cost: There is a modest fee of $3.50 per hour

Helpline
Helpline is a telephone reassurance service for isolated or homebound seniors. The program provides human contact with a daily reassurance call, letting clients know that someone is there for them. Many of the volunteer callers are senior citizens themselves. Helpline volunteers reach out to homebound seniors with a friendly voice—asking how they’re doing, answering questions, providing information about Town or County services or just lending an ear— from one person to another. Calls are made from the Town’s recreation and nutrition centers in 2 different shifts: 10 am to Noon and 1 pm to 3 pm.
Cost:
There is no charge for the Helpline program.
